OUR NEW ANNUAL MEMBERSHIP PROGRAM

• WomenFirst, is a Membership Program that is designed
to provide compassionate, one-on-one, unhurried
care by me, Holly W. Hadley, MD.

• The Relationship is between you, the patient, and me, your doctor. The insurance company is not intruding on that relationship.

• Together, our goals are to create a personalized wellness program and to detect medical problems early with
immediate follow up treatment customized for you.

For an Annual Fee you will receive the following benefits:

• Uncomplicated access to great healthcare.

• Same day, on time appointments.

• Office visits that allow you as much time with me as you need to discuss problems and health concerns.

• A Comprehensive Annual Physical Examination including GYN exam, EKG and Bone Density Testing.

• Direct contact with me by phone after hours if necessary.

• Educational Sessions with me as they apply to your medical concerns.

• On-site laboratory services.

My billing for additional office visits will be:

• As a non-participating provider for all insurances except Medicare. I accept Medicare assignment.

• Reduced office visit fees where applicable

• My office will continue to handle all billing to your insurance company.
